Health & Dental Insurance:
Employees who meet the required FTE level will qualify for health and dental insurance coverage. The Hospital will pay 85% of the single premium for health insurance and 50% of the premium for family coverage. The employee is responsible for the remainder of the premium through a payroll deduction. The employee pays a portion of single dental coverage and is responsible for the entire family premium for dental coverage.
Life Insurance:
Employees who meet the required FTE level qualify for Hospital paid term life insurance. Coverage is one times annual base salary (per FTE) with a minimum of $15,000 coverage and a maximum coverage of $40,000.
Short Term & Long Term Disability Insurance:
Employees who meet the required FTE level are eligible for hospital paid Short Term Disability insurance coverage. The employee is insured for 60% of his base wage (per FTE) for 24 weeks. Coverage commences on the 29th day of his/her disability. Long term disability coverage may be purchased by the employee.
Retirement Plan:
All Hospital employees are eligible for the Hospital’s 401k Retirement Plan at the time of hire. When the employee has completed 1 year of service and worked a minimum of 1000 hours, he/she will be eligible to receive 50% up to 4% matching of their pension deferral. The plan is through John Hancock.
Tax Deferred Annuities:
Individual 403b tax deferred annuity plans are available to all employees through a payroll deduction.
Paid Time Off:
All employees (with the exception of casual and temporary employees) are eligible for paid time off (PTO). Accrual rates are per contract agreement or per Hospital policy.
Family & Medical Leave Act Leave:
The employee must be employed for at least 12 months and worked at least 1250 hours in that time to qualify for this unpaid leave. The Hospital follows the guidelines of the Family & Medical Leave Act of 1973. Employees must request this leave 30 days in advance of leave whenever possible. Employees absent for 3 consecutive scheduled shifts of work due to his/her own serious health condition will be automatically placed on F&MLA Leave. The Hospital policy on this leave is available in the Human Resource office.
Sunshine Club:
All employees are eligible to join this club. The cost is $25.00 per year.

This handbook for employees is available through the Human Resource office. It answers many of the questions you may have concerning your employment here at Falls Memorial Hospital.
Series I Bonds:
Series I Bonds are available to all employees. Sold at face value, (you pay $50 for a $50 bond) these bonds offer a real rate of return over and above inflation. New rates are announced each May 1st and November 1st. Bonds may be redeemed anytime after 12 months. Bonds are exempt from state and local income taxes.
